Summary Annual Reports

Under the terms of the Employee Retirement Income Security Act (ERISA), it is required that a summary annual report for Duke's welfare benefit plans and the Duke Faculty and Staff Retirement Plan be furnished to plan participants and beneficiaries. These reports are available below. A printed copy of these reports may also be obtained, at no cost, by contacting the Human Resource Information Center at (919) 684-5600.

Summary of Material Modification

This notification serves as a Summary of Material Modification ("SMM") to the Duke University Welfare and Fringe Benefits Plan. This SMM modifies and supplements the benefits described in the official Summary Plan Description (SPD) for these plans.
