LIVE FOR LIFE Personal Health Coaching
Coaches work in partnership with you 1-1 to achieve a higher level of physical and emotional well-being, especially in areas where making change is hard. The coach will work with you to discover and clarify the goals that you wish to achieve, and will help you facilitate changes that you choose to make over a period of time. Through collaboration with a coach, you will find solutions and strategies to making changes, and an accountability partner to hold you responsible for the choices you make.
Common health goals are:
- Improve sleep
- Manage stress
- Lose Weight
- Lower cholesterol or blood pressure
- Improve blood sugar control
- Improve eating habits
- Improve exercise habits
During this 12-week program, you will meet for an initial coaching session for 60 minutes, with subsequent sessions lasting 15-20 minutes. Typically, sessions occur weekly, and then taper to bi- monthly or monthly when the participant is ready. Most sessions will occur via ZOOM in a private meeting room with your coach. Once established, sessions may occur via phone, email, zoom, or on occasion, in-person meetings can be established with your coach.

All participants will be asked to submit recent biometric data from a doctor's visit periodically. This includes height, weight, blood pressure, cholesterol numbers, and fasting blood sugar/A1c.
LIVE FOR LIFE health coaches hold a degree in a health care field such as nursing, dietetics, or exercise physiology. Our coaches also achieve additional training and certification in health and wellness coaching.

LIVE FOR LIFE Group Health Coaching
Join LIVE FOR LIFE for a Group Health Coaching Program. This program provides support and education to help you achieve and maintain self-care practices that increase your overall health and well-being. Set personal goals and action steps for personal growth areas, including nutrition, exercise, better sleep, mental clarity, stress management, and life balance.
Through this comprehensive and integrated approach, you will stay motivated and accountable, learn vision-setting and mindset practices, discover ways to overcome obstacles and challenges, and develop systems that support your personal growth in a peer support setting.
Enrollment is limited to no more than 8 individuals per group. During the enrollment period, enrollees will meet with their coach one-on-one for 45 minutes to establish goals and priorities for the upcoming program. During part one of the program, participants meet for 4- 45-minute weekly group sessions. Your coach will schedule a second check-in session with you after the first four sessions, which will be followed by 4- 45-minute group sessions every other week. Your coach will meet with you for a final coaching session to wrap up and plan for the future. The full program lasts for approximately 3 months.
Enrollees will be asked to pay $25 fee to hold their slot in the program. The fee is refunded at the end of the program for those who attend at least 6 group sessions and all one-one sessions with their coach. Pathways to Change (Pre-Bariatric Procedure Health Coaching)
If you are a benefits eligible employee or dependent with a Duke insurance plan, you may choose to enroll in Pathways to Change, our pre-bariatric procedure health coaching program. We will work with you for at least 3 months prior to your procedure and will continue to support you with lifestyle changes throughout the following year. Program start is typically initiated after your first visit to the Duke Center for Metabolic and Weight Loss Surgery but is available for enrollment at any time. The program requirements must be completed no more than 6 months prior to the date of surgery. For enrollment instructions, please contact Sally Neve, RD, at 681-0525. Other requirements apply. Tobacco Quit Coaching
Are you, or someone you love, ready to use less tobacco or quit using tobacco? Components if this program include:
- Copay-free visits with our provider for consideration of prescribed medication therapies.
- Low-cost nicotine replacement therapies such as gums, lozenges, or patches.
- Quarterly follow up visits with a tobacco quit coach (in person or telephonic sessions are available).
