Please note that the LIVE FOR LIFE Duke Fitness Club contract requires at least a three-month commitment to a gym. You must fulfill this time commitment before you can cancel your membership.
Life Time Fitness Memberships require a 30-day written notice to the club in addition to the LIVE FOR LIFE cancellation form. Visit the Life Time Fitness page to learn more about this process.
To cancel membership with the Duke Fitness Club, please complete the following steps:
Step 1 - Complete an Online Cancellation Form
To cancel a membership from a fitness facility, access the Cancel/Change Membership website, select the facility that you would like to cancel, and send a cancellation. Your privacy is important to us so this website has been secured and will require your Duke NetID. You may also need to be logged into the Duke network or VPN to access this site. If you have questions or encounter any operational issues with this website, please call LIVE FOR LIFE, and we will help you.
Step 2 - Confirmation
Once submitted, you will receive an email that confirms our receipt of your cancellation request and a copy of our cancellation policy. If you email has changed or you do not receive your confirmation, please let us know so we can forward you a copy of the form. Should you have any questions, please contact a LIVE FOR LIFE staff member at 919-684-3136, option 1.
LIVE FOR LIFE is not responsible for cancellations submitted directly to the gym. To end your membership and payments, you must start by filling out an online cancellation form at the link above.
Note: You may also cancel by calling LIVE FOR LIFE. Voicemail and/or email cancellation requests are not an accepted form of notification. To help us assist you, please provide us with your Duke Unique ID number, your first and last name, the gym you are cancelling, and how we can best reach you to confirm your request.
