Loan Forgiveness Forms

Employees may be eligible for student loan forgiveness under several different programs such as the NC State Education Assistance Authority (NCSEAA) and the Public Service Loan Forgiveness Program (PSLF). The Public Service Loan Forgiveness (PSLF) program can forgive the remaining balance on Direct Loans after you make 120 qualifying monthly payments under a qualifying repayment plan while working full-time for a qualifying employer (such as U.S. government agencies, the military, public schools, universities, or certain nonprofits).

Duke University & Duke University Health System have partnered with Equifax’s The Work Number to assist current and former employees with the completion of these loan forgiveness forms. The Work Number® PSLF Form Fulfillment feature will help you complete the PSLF form more easily by using your payroll data to pre-populate your information.

Completing a Loan Forgiveness Form via The Work Number

Log into theworknumber.com.
Register Now to create a new account
Go to the Letters Center through the top menu bar or by clicking on the Letters Center tile.
Click on the word Generate or on the arrow icon next to the type of letter you want to generate.
Review and validate pre-populated data, click Next if correct

Note: If you edit any personal or employment data, the employer’s electronic signature will be removed, and you’ll need to get a manual signature from your Department Payroll Representative.

You can locate your Payroll Representative on the Corporate Payroll website or via Duke@Work self-service website: My Info > My Profile >Find My Payroll Representative.

Once signed, download the completed PSLF form
Submit form to your loan servicer or through the PSLF Help Toolon gov/pslf to start the certification process
