Health Review for Animal Handlers
If you have computer access along with a Duke netID (this is what you use to do the training modules) please complete and submit the Web form of the Animal Handler Health Review. This way is the fastest and most secure.
If you do not have access to a computer or are having VPN issues from home computers (for example, you receive and "error: the site can't be reached" message when trying to connect) you may print and complete the PDF form of the Health Review and fax it to EOHW at 919-681-0555 or email it to animalhandler@dm.duke.edu. Please call Employee Occupational Health at 919-684-3136, option 2 if you have any questions and a member of the EOH staff will assist you.
