The Balance Blueprint Diabetes Prevention Program is a structured, evidence-based wellness program designed to help individuals at risk for type 2 diabetes build sustainable, healthy habits. Through a balance of personalized nutrition guidance, physical activity strategies, and behavior change coaching, participants learn practical skills to improve blood sugar control, support weight management, and reduce long-term health risks. The program emphasizes small, achievable steps, self-monitoring, and supportive accountability to empower lasting lifestyle change.
Program Structure and Details
- Open to all Duke benefits eligible employee’s or their dependents at least 18-years of age.
- Recent Hemoglobin A1C (HbA1c) measure between 5.7% to 6.4% (diagnostic of pre-diabetes).
- Attend a weekly group health coaching session (held virtually) for 10-weeks.
- Attend a one-one session with a Dietitian at the beginning of the program (held virtually).
- Complete a weekly accountability and planning survey each week.
- Cost is $25 to enroll, with fee refunded for positive attendance (80% or more sessions attended).
