Step 1 - Cancel Current Membership
Complete the online cancellation form. Please be aware that payroll deduction may not cease for at least four weeks due to the payroll schedule.
** The LIVE FOR LIFE Duke Fitness Club contract requires at least a three-month commitment to your current gym prior to cancelling.
Step 2 - Enroll in a New Facility
Enroll in a new facility by completing the online registration form. Payment is arranged for your convenience through payroll deduction, but depending on the payroll cycle, one or two months payment upfront may be required to begin your new membership immediately.

Step 3 - Submit Completed Forms
Once submitted, you will receive an email that confirms our receipt of your cancellation request and a copy of our cancellation policy. Should you have any questions, please contact a LIVE FOR LIFE staff member at 919-684-3136, option 1.
