Enrollment Instructions

As a new staff member, you have 30 days from your date of employment to enroll in certain benefits, including medical, dental, vision and reimbursement account plans. Make your benefit selections carefully as you will not be able to make changes afterward until the next annual open enrollment period, unless you have a qualifying life event.

You may enroll online through the Duke@Work self-service website or return completed enrollment forms to the address listed on each form. For your convenience, a checklist of benefits for which you may be eligible is listed below.

Benefit Plan Web or Form How to Enroll
Medical, Dental and/or Vision Web* You have up to 30 days after your hire date to enroll in these plans. Otherwise, you can only enroll after a qualifying life event or during the annual open enrollment period.
Reimbursement Accounts Web* You have up to 30 days after your hire date to enroll in the reimbursement accounts. Otherwise, you can only enroll after a qualifying life event or during the annual open enrollment period.
Personal Accidental Insurance Web*
Form (after 30 days from hire date)
You can enroll in this employee-paid plan online within the first 30 days after your hire date. Otherwise, you can enroll anytime using the paper form.
Basic Life Insurance Web* Please designate your beneficiary(ies) online immediately.
Voluntary Disability Insurance Form If you enroll within the first 30 days, you do not have to answer medical questions. Return the completed form to the address listed on the form.
Supplemental Life Insurance Form
Beneficiary Form
Faculty and Staff Retirement Plan See options on web You may enroll in any of these plans at any time. Please refer to the links to the left for more details.
Auto and Home Insurance Vendor website

*Duke@Work is a secured self-service website that is accessible once your hire form has been processed and you have been issued a NetID and password (typically within 24-48 hours after the hire form has been processed). Please check with your hiring manager if you have not yet been issued a NetID and password.
