All Duke University staff are required to complete and submit the Redeployment Agreement Form to be eligible for redeployment to support critical clinical operations during the COVID-19 pandemic. If you do not have a means of scanning the completed form, you may take a picture of the completed form and submit a clear picture of the form. Forms should be submitted to this Qualtrics website.
Once your completed form has been received, the Intake Coordinator will be notified of your approval to be redeployed and will contact you regarding next steps.
