The cost for your coverage is based upon your annual base salary and age. Refer to the Premium Rate Table to determine your monthly premium. The deduction amount will automatically change each January, based upon your age and your annual base salary changes as well as any change to the Premium Rate Table.
The cost of insurance rates are adjusted on the January 1 program anniversary to reflect the age of the insured adult. The following rates are effective Jan. 1, 2014:
