Supplemental Payment Form
Replaces the Personal Service Requisition. Form used for exempt employees to issue supplemental
payments in addition to the base salary. This form should be used for exempt
employees only.
The Supplemental Payment Form should be sent to the HR Information Center at 705 Broad Street (Box 90496), Durham, NC 27705 (fax number 919-668-0386)
after approval by the appropriate departmental and/or budgetary official.
Please Note: Supplemental Pay Forms for
some job codes should be sent directly to Corporate Payroll Services
at 705 Broad Street (Box 90484), Durham, NC 27708 (fax number 919-668-5840)
after approval by the appropriate departmental and/or budgetary official.
