Employee Orientation Sign-Up

This form is to be used by payroll clerks or managers to register employees in the orientation welcome event. All employees are required to attend orientation. Employees must enroll in health insurance, dental insurance, vision insurance, and reimbursement accounts within 60 days of gaining eligibility. Please fill out the form completely.
