Placement Health Review

• What: Prior to beginning work after job offer employee must complete
• Why: To assure that employee can safely work in position
• Who: Most positions in hospital, many positions in medical center, and some positions on campus
• How: Contact EOHW at 684-3136
• Where: EOHW Duke Clinic (South), Room 04290 Red Zone, PRT Level

Periodic Surveillance Health Review

• What: Periodic review of health status
• Why: To assume that safety programs are accomplishing their goal and that employee can continue to safely work in position
• Who: Many of the positions that require a placement health review
• How: Employee and supervisor notified by EOHW
• Where: EOHW Duke Clinic (South), Room 04290 Red Zone, PRT Level
