What is the Steps to Health program?
Steps to Health is an incentive program offered by LIVE FOR
LIFE® that focuses primarily on helping you achieve measurable
health results and rewards you for achieving your health goal.
Through the Steps to Health program, you will have the opportunity
to make realistic and measurable changes to your health by
creating a step-by-step plan for healthier living with guidance
from a LIVE FOR LIFE staff member.
You will earn LIVE FOR LIFE dollars through Steps to Health
by participating in quarterly measurements and achieving your
predetermined health goals. As you progress in the program,
the LIVE FOR LIFE dollars awarded to you will increase in
value. You have the potential to earn up to $1000 LIVE FOR
LIFE dollars that can be used to purchase merchandise from
the LIVE FOR LIFE store.
Can I still participate in the Lifestyle
Activity Record program?
On October 1, 2004, Steps to Health replaced the Lifestyle
Activity Record as LIVE FOR LIFE’s health incentive
program. The Lifestyle Activity Record was introduced in the
late 80s to encourage faculty and staff to make a positive
impact on their health by practicing healthy lifestyle behaviors.
Steps to Health represents a new and improved approach to
achieving the same goal by incorporating what we have learned
since introducing the LAR program.
You will still have the opportunity to earn LIVE FOR LIFE
dollars through Steps to Health program by participating in
quarterly measurements and achieving your predetermined health
goals.
Why did you transition from the Lifestyle
Activity Record program to Steps to Health?
LIVE FOR LIFE is dedicated to optimizing the health, well-being
and productivity of Duke faculty and staff. Steps to Health
is an incentive program that focuses primarily on helping
you achieve measurable results and rewards participants for
achieving their health goal. By introducing the Steps to Health
program, we are offering you more valuable health services
by focusing on measurable health results.
How does the Steps to Health program work?
Step 1
To participate in Steps to Health, you will first need to
participate in a HealthCheck, a free and confidential health
assessment administered by a LIVE FOR LIFE nurse. HealthCheck
gives you the opportunity to have your cholesterol, blood
sugar, blood pressure, height, and weight measured and your
body mass index calculated. In addition, you will be asked
to complete a Health Risk Assessment (HRA) questionnaire about
current lifestyle and habits regarding physical activity,
nutrition, smoking and stress management. Upon completion
of the HealthCheck and HRA, you will receive your first incentive
reward of $200 LIVE FOR LIFE dollars.

Step 2
After completion of the HealthCheck, you will meet with a
LIVE FOR LIFE staff member who will discuss the results of
your HealthCheck and HRA, assess your current health, and
identify any specific health issues you may face now or in
the future. You and your LIVE FOR LIFE coach will discuss
your personal health needs and set measurable goals to work
towards in areas such as weight management, blood pressure,
stress, cholesterol, smoking, physical
inactivity or health maintenance.
Step 3
Throughout the next 12 months, you will receive communications
from your LIVE FOR LIFE coach via e-mail, phone and in person.
He/she will provide interactive sessions to assist with setting
realistic, measurable changes and achieving your health goals.
During this time, you can record your measurements on a Healthy
Tracker Card or enter data into eHealth, a personal wellness
web site, where you can track your measurements and goals.
As a participant in the Steps to Health program, you will
earn LIVE FOR LIFE dollars for participating in quarterly
measurements and achieving your predetermined health goals,
which can be used to purchase merchandise for the LIVE FOR
LIFE store
.
Step 4
After 12 months in the program, you will complete a second
HealthCheck. If your predetermined goal is achieved, you will
receive the LIVE FOR LIFE dollars incentive reward and the
personal satisfaction of achieving your goal and becoming
a healthier you. You are encouraged to continue in the program,
choose another goal, maintain your health measurements and
earn additional LIVE FOR LIFE dollars. If you have not reached
your goal, you can work with LIVE FOR LIFE to continue in
the program until your goal is achieved.
Who is eligible to participate in the
Steps to Health program?
LIVE FOR LIFE is a contracted service and available to most
Duke employees. LIVE FOR LIFE is not available at Durham Regional
and Duke Raleigh Hospital. Durham Regional employees
can call 470-8453 for information on their health promotion
program.
What if I have recently participated in
a HealthCheck? Will I need to participate in another in order
to enroll in the Steps to Health program?
If you have participated in a HealthCheck within 3 months
of signing up for the Steps to Health program, you will not
be required to participate in another HealthCheck to enroll
in the program. You may register for the Steps to Health program
by contacting LIVE FOR LIFE at 684-3136, option 1.
If it has been more than 3 months since you participated
in a HealthCheck and you would like to participate in the
Steps to Health program, you will need to participate in another
HealthCheck.
Is the HealthCheck a part of Duke Prospective
Health?
If you are a Duke Select or Duke Basic member, you are eligible
to participate in Duke Prospective Health. As a partner of
the Duke Prospective Health program, LIVE FOR LIVE will send
your HealthCheck information to Prospective Health, which
takes care of the Health Risk Assessment required for the
program. All of the personal and health information remains
confidential among the Duke Prospective Health clinical care
team, you, and your physician.
How can I earn LIVE FOR LIFE dollars through
Steps to Health?
You will earn LIVE FOR LIFE dollars through Steps to Health
by participating in measurements and achieving your predetermined
health goals. As you progress in the program, the LIVE FOR
LIFE dollars awarded to you will increase in value. You have
the potential to earn up to $1000 LIVE FOR LIFE dollars that
can be used to purchase merchandise from the LIVE FOR LIFE
store.
What type of merchandise will I be able
to purchase with my LIVE FOR LIFE dollars?
Items such as fitness videos, embroidered sweatshirts, golf
shirts, pedometers, fleece jackets, duffle bags and much more
will be available through the LIVE FOR LIFE store. We will
be evaluating our current merchandise offerings and use feedback
from current participants to provide additional incentive
items.
The merchandise will be on display in front of the Employee
Occupational Health & Wellness office located at 04290
Duke Clinic, Red Zone, Monday-Friday from 8:00 a.m.-5:00 p.m.
to do your shopping! (Please note: Our reception area is closed
Wednesdays from Noon – 2 p.m.)
Can I use my LIVE FOR LIFE dollars for
massage therapy?
Yes, LIVE FOR LIFE dollars distributed through the Steps to
Health program will be accepted as payment for a massage.
Massage appointments are available on Thursday's from 1:30
– 4:45 p.m. Call 684-3636, option 1, to schedule a 15
or 30 minute massage.
Can I use LIVE FOR LIFE dollars earned
through the Lifestyle Activity Record program in the Steps
to Health program?
YES, LIVE FOR LIFE dollars earned through the LAR program
will be accepted in the Steps to Health program.
What is a Healthy Tracker Card?
The Healthy Tracker Card is a paper form used to track your
personal health measurements as it relates to your Steps to
Health program goal. This form can also be used as a reminder
of your health measurements and serve as a motivational piece
to continue with the program. Visit the Steps to Health web
site at <www.hr.duke.edu/eohs/livelife/incentive.html>
to download and print the Healthy Tracker Card and begin tracking
your personal health measurements.
Can I track my goals online?
As a participant with internet access, you can utilize eHealth
to track your measurements online.
What is eHealth?
eHealth is a free, personal wellness web site offered by LIVE
FOR LIFE that enables Duke faculty and staff to track their
progress toward their health and fitness goals online.
eHealth offers a range of features including personalized
home pages, health trackers, and health information that can
help employees make effective decisions about their health
care. You will also find automated daily health calendars,
online diaries, and cardio and workout logs where you can
keep track of important health appointments and information
.
How do I begin using eHealth?
- Visit www.hr.duke.edu/ehealth
- Click on “Login?” on left side of page
- Follow the instructions for Sign In
- Use your 8-digit Unique ID as your User name
- If 8 digits, correct
- If only 7 digits, add an extra zero to front of number
- If number starts with 9, omit the 9
- Use same 8 digit ID for password
- Change your password after first time signing in
- You are all set, just be sure to write your User
name and password in a safe place for next time!

What are the indicators of success with the program?
