Health Insurance

You can get a listing of participating health care providers at the HR Information Center, 705 Broad Street, or online at:

• Duke Select / Duke Basic Members (scroll down and click on the Duke logo)
• Blue Care Members
• Duke Options Members

Dental Insurance

Duke has an expanded network of providers who have agreed to accept usual and customary (U&C) for covered services. Under the dental plans, participants are able to visit any licensed dentist. You can find a network provider by calling Ameritas at 1-800-487-5553 or by visiting the Ameritas web site.

Vision Care Insurance

To find a network provider, please visit the UnitedHealthcare Vision web site or call 1-800-638-3120.
