Health Management Programs
 Nearly 80 percent of disease in this country is preventable. Preventive
health is one of the best ways to reduce claims and combat the rising cost of
health care.
Practicing Prevention
The costs of Duke's health plans are directly related to the health and pharmacy claims
of those covered by the plans. Higher claims mean increased cost for both Duke and
covered participants. Duke covers the majority of the cost for health care, but an
increase in claims also means an increase in monthly premiums for faculty and
staff. One of the best ways to reduce claims cost and the rising cost of health
care is by practicing prevention.
You may have heard that "an ounce of prevention is worth a pound of cure." Today,
with nearly 80 percent of disease in this country being preventable, this advice has
never been more relevant. Duke supports you and your family in practicing prevention
through a broad range of health management programs that emphasize a proactive
approach to improving health and preventing disease before it happens.
Duke Prospective Health
 Since participating in Duke Prospective Health, Faydean Cannada has lost 20 pounds, and she's eating healthier and walking
regularly.
Her story is one of many across Duke that reflects the success of a program launched in 2004 to identify and reduce
health risk behaviors.
As research and medical science advance, the likelihood of predicting
who is at risk for developing certain diseases moves further away from
the world of fiction and Hollywood and closer to the world of your
local doctor's office. Through programs such as Prospective Health,
Duke is on the forefront of bringing these advances directly to you.
Duke Prospective Health, a program for those enrolled in either
Duke Select or Duke Basic health care plans, is designed to
identify an individual's risk for debilitating diseases and chronic
conditions — such as heart attack, hypertension and diabetes —
before they occur.
The program helps you work more closely with your personal
physicians to identify your health risks. The program then helps
you create a step-by-step plan for your optimal health and
provides support and resources to help you achieve your goals.
Prospective Health is a voluntary and confidential program,
which means you control your personal health information
and who has access to it.
Depending on the specific health care needs, participants
may receive support that includes:
- a health care coach, who works with a group of participants on specific issues
- a care manager, who works one-on-one with participants and coordinates support from a team of health care professionals
- integration into programs coordinated by LIVE FOR LIFE®, Duke's health and wellness program.
The program also offers incentives, such as discounts for prescription drug
co-pays, for individuals who qualify for additional services and take an active

LIVE FOR LIFE
 After turning 40, Aranjeet Sidhu, a clinical staff pharmacist at Durham Regional Hospital, wanted to get in shape
and in the process, find a fitness club that would benefit her as well as her family.
As part of Duke's commitment to promote and support healthy lifestyles, eligible
faculty and staff* can participate in a variety of wellness opportunities offered by LIVE
FOR LIFE, Duke's employee health promotion program. Following are some of the
programs and services offered by LIVE FOR LIFE to help you reach your health and
fitness goals:
Health Assessments and Education, including assessments of blood pressure,
cholesterol, height, weight, and body composition and programs to reduce health risk
factors such as stress and smoking
Fitness Activities, including discounted memberships to fitness facilities throughout
the Triangle, consultations with fitness specialists, and group run/walk programs
Nutrition Activities, including personal consultations with a registered dietitian for
ways to improve your health through your diet and self-paced programs to help you
achieve your weight loss goals and overall wellness

24-Hour Health Care Advice Line
Ever wonder what to do if your child becomes ill in the middle of the night? Do you
have questions about a new medication you are taking? The answers are only a phone
call away. Twenty-four-hour health care advice lines are available under all of Duke's
health care plans. By calling a health care advice line, you can get confidential, up-todate
health information any time of the day or night. Professional staff are available
to assist you with almost any medical question, offer support, or help you navigate
the health care system.
Duke Select and Duke Basic members can call the 24-Hour Health Advice Line at
1-888-662-2296. Duke Options and Blue Care members can call the Health Line Blue
24-Hour Health Information Line at 1-877-477-2424.
Blue Extras Program
For participants of Blue Care and Duke Options health plans, Blue Cross Blue Shield
offers a range of free health management services and programs through the Blue
Extras program to help you take charge of your health and save money.
Blue Extras offers incentives and special benefits such as free reference books, medication
organizers, and glucometers to members. You can also earn rewards through the BluePoints
incentive program by keeping track of your wellness activities. In addition, discounts are
available on health services such as acupuncture and chiropractic services and products such as

Personal Assistance Service (PAS)
Personal Assistance Service (PAS) is the faculty/employee assistance program at Duke. It has expanded the number of free visits
